Initial contact: If you initiate contact via the website’s consent form you provide your name and email address. This information is then sent to the company email address. By completing the form you consent to being contacted in response to your query. If you do not respond to any contact attempts or you decide you do not wish to continue contact, your details will not be retained.
You may opt to contact me by phone, text, email or through social media. A record of this contact, in its’ original form, is kept whilst correspondence or providing therapy is ongoing. Therefore information may be stored within the email system, on a password protected mobile phone or in social media messaging systems. By getting in touch you consent to being contacted in response to your query. If you do not respond to any contact attempts or you decide you do not wish to continue contact, your details will not be retained.
Storage of personal information: Personal information will be stored for the purposes of providing therapy services. This may include name, phone number, address, email address and other personal details.
The information will be stored securely, for example in password protected documents on password protected devices. Or it may be stored within email/phone/social media accounts where these devices will be secured with passwords.
Any information gathered via paper form will either be stored securely in a locked facility or be destroyed appropriately once converted to an electronic record. Records from accessing the service will be kept either electronically or by paper. Where possible, paper records will be held without personal identifiable information.
Information retained for booking services: In order for you to access the service, I need to store information about you to arrange appointments. This information will include your name, email address, and phone number, and any other information you have provided to us. This information will be used solely to provide the service.
Information retained for completing services: If services are accessed, records of this will be kept for financial reasons (invoicing) and for medical and legal reasons. Notes of all therapy session are required to be kept. Records will be stored securely and kept for a period of reasonable time (typically 7 years). After which time they will be destroyed securely. You have a right to access your personal data, if you wish to request this please do so via ialfordcounsellingservices@gmail.com
Third parties: Your information will not be sold to third parties. Your information will not be shared with third parties for marketing or research purposes.
If you are accessing the service funded by a private company or health insurance, we are required to share information with them. By accessing the service via these methods, users are consenting to this sharing of information.
All therapists are required to have clinical supervision and discuss clients in these sessions. Client information may be discussed, but you will not be identifiable.
If we are provided with information that suggests that you or someone else is at immediate risk or harm, we are required by duty of care to contact relevant services. This may be done without your permission. This could include contact with emergency services, your GP or other healthcare professionals. I would always endeavour to speak to you about this first and gain your consent.
